We are currently seeking a compassionate, experienced, and a highly organized client care coordinator to join our growing team. In this role, you will play an important part in our ability to provide exceptional care and a remarkable experience to our customers by assessing client needs, managing, coaching & scheduling our caregivers, and providing support for the clients & caregivers we serve.

The Care Coordinator (CC) is responsible for ensuring a high end, professional care experience to our clients who need assistance with daily activities by scheduling existing caregivers to any open shifts, covering new cases as they come in, and any new open shifts as caregivers call off. The CC is equally responsible for managing the caregiving team who provide this assistance to the clients. This includes scheduling shifts, ensuring that caregivers have the proper training and are equipped to handle their responsibilities with their assigned client, cultivating strong relationships with the client and his or her family in order to understand their needs and concerns, communicating regularly with the Director of Care Coordination & Executive Director and other Senior1Care team members about the client’s progress and needs. The CC will creatively look for ways to schedule & influence clients & caregivers. Maintain client/caregiver reminders as set to licensure & labor standards.

Duties and Responsibilities

· Ensure that our clients are receiving the very best in care

· Manage and oversee the caregiver team

· Match clients with caregivers based on client needs, personality & location

· Communicate effectively with caregivers, clients, families, and referral sources

· Manage all client schedules, ensuring care plans are being met

· Maintain accurate client documentation

· Manage caregiver schedules and requests for time off

· Assign caregiver trainings

· Maintain compliance with all legal and company requirements at all times

· Answer incoming calls and be on the night/weekend on-call rotation

· Support caregivers, clients, and families by providing accurate information & resources when necessary

· Maintain a compassionate demeanor at all times

· Provide innovative feedback to improve processes & the quality of the care

· Remain calm and collected in difficult situations

· See that all measures are taken to protect client privacy and dignity

· Intake & develop client care plans and see that they are properly carried out

· Document and report any issues or violations immediately

· Make oversight visits to supervise and assess clients

· See that client care notes are maintained and monitor client quality care/change in conditions

· Screen potential caregiver applicants (phone and in-person) with general questionnaire and reporting to Talent Acquisition Manager 

· Work to foresee challenges and implement proactive solutions

· Demonstrate ability to be a problem solver

· Collect feedback to ensure client satisfaction

· Provide information to prospective clients and follow up with them

· Participate in interviewing, hiring, evaluation, and discipline processes of caregivers

  · Bachelor’s degree in a relevant field, additional relevant certifications a plus

· Experience in healthcare, mental health field, and/or homecare preferred

· Minimum 2 years’ experience as a care coordinator or other relevant role

· Solid computer skills, including Microsoft Office

· Familiarity with office equipment and phone systems

· Excellent phone etiquette

· Able to diffuse conflicts and disagreements, then find a workable solution
